Service invoice example
Header: Your name or business, contact info, “Invoice,” number (e.g. INV-001), date, due date (e.g. Net 30).
Bill-to: Client name, address.
Line items:

Same idea: clear description, quantity, rate, amount, then subtotal and total. Add tax if it applies.
Freelancer example
For hourly work, list the task or period and the hours:
